What it checks
Measures Apolipoprotein B, the protein carried by every particle that can clog an artery — LDL, VLDL and Lp(a). Because each of those particles carries exactly one Apo B, the result counts how many harmful particles are circulating, which often predicts heart risk better than LDL cholesterol alone.

What is a Apolipoprotein B (Apo B), Serum ?

Included Tests

Apolipoprotein B (Apo B), Serum — a single blood test that counts the cholesterol-carrying particles in your blood capable of building up inside artery walls.

Details

Know More About This Test

Apolipoprotein B, almost always shortened to Apo B, is a protein that sits on the surface of certain cholesterol particles in your blood. Those particles — LDL, VLDL, IDL and Lipoprotein(a) — are the ones capable of pushing into an artery wall and starting the slow build-up that narrows arteries over many years. Particles that protect you, such as HDL, do not carry Apo B at all.

The useful part is the arithmetic. Each of these particles carries exactly one Apo B molecule. One particle, one Apo B. So when a laboratory measures the total Apo B in your blood, it is effectively counting how many harmful particles are circulating. No other routine blood test gives you that number.

This matters because a standard cholesterol report tells you about weight, not headcount. LDL cholesterol measures how much cholesterol is being carried inside those particles. Two people can return an identical LDL figure while one of them is carrying far more, smaller, denser particles than the other. It is the number of particles knocking against the artery wall that drives the damage, not the amount of cholesterol packed inside them. That is the gap Apo B fills.

For this reason many cardiology guidelines now treat Apo B as a more reliable marker of cardiovascular risk than LDL cholesterol alone, particularly in people with diabetes, obesity, high triglycerides or metabolic syndrome — exactly the groups where the LDL number is most likely to be misleading.

Why Is This Test Recommended?

Your doctor may suggest an Apo B test in order to:

  • measure cardiovascular risk more precisely than a routine cholesterol panel allows
  • make sense of a lipid profile where the numbers do not tell a consistent story
  • assess risk in someone whose LDL cholesterol looks acceptable but who has other warning signs
  • investigate risk where there is a family history of heart attack or stroke at a young age
  • check how well a statin or other cholesterol-lowering treatment is working
  • provide one half of the Apo B / Apo A1 ratio, a widely used summary of cardiac risk
  • build a fuller picture during a yearly or preventive health check

Why a Particle Count Can Differ From a Cholesterol Number

Picture two depots sending out the same total load. One sends ten large trucks; the other sends thirty small ones. The tonnage is identical, but thirty vehicles cause far more congestion than ten. Cholesterol particles behave the same way. Small, dense LDL particles carry less cholesterol each, so more of them are needed to move the same load — and every extra particle is another chance for one to lodge in an artery wall.

People with insulin resistance, type 2 diabetes, high triglycerides or excess weight around the middle tend to produce exactly this pattern: many small particles rather than few large ones. Their LDL cholesterol can read perfectly normal while their particle count is high. This situation is common enough to have a name — discordance — and it is the single most important reason Apo B is ordered.

Apo B and LDL Cholesterol Compared

  • LDL cholesterol tells you how much cholesterol the harmful particles are carrying.
  • Apo B tells you how many harmful particles there are.
  • Non-HDL cholesterol sits between the two and is a useful estimate, but it still measures weight rather than number.
  • When the two disagree, research consistently finds the Apo B figure tracks actual risk more closely.

None of this makes a lipid profile redundant. The two are read together, and most doctors will want both.

What Do the Results Indicate?

  • A high Apo B result means a larger number of artery-penetrating particles are in circulation. This is associated with greater long-term risk of narrowed arteries, heart attack and stroke. The higher the count, the greater the concern, particularly if it persists across repeated tests.
  • A result in the expected range suggests the particle count is not adding to your cardiovascular risk. It does not clear you of risk from blood pressure, smoking, diabetes, weight or family history, all of which act independently.
  • A low Apo B result is generally regarded as favourable, and is often seen in people already on effective cholesterol-lowering treatment.

Your report will carry the reference range used by the laboratory. Ranges differ slightly between laboratories and testing methods, so always read your result against the range printed on your own report rather than one found online. These results are not a diagnosis on their own. Your doctor will read them together with your cholesterol profile, blood pressure, blood sugar, weight, smoking history and family history before deciding what, if anything, needs to change.

The Apo B / Apo A1 Ratio

Apo B is frequently ordered alongside Apolipoprotein A1, the main protein in HDL. Apo B counts the particles that damage arteries; Apo A1 reflects the system that clears cholesterol away from them. Dividing one by the other gives the Apo B / Apo A1 ratio — a single number capturing the balance between harm and repair.

A higher ratio points to higher cardiovascular risk. Large international studies have found this ratio to be one of the strongest single predictors of heart attack available from a blood sample. Calculating it requires both tests, which is why they are usually booked together.

What Can Push Apo B Higher

  • an inherited tendency towards high cholesterol, including familial hypercholesterolaemia
  • a diet heavy in fried food, refined carbohydrates, sugar and trans fats
  • type 2 diabetes, prediabetes or insulin resistance
  • excess weight, particularly around the abdomen
  • very little physical activity
  • an underactive thyroid, kidney disease or liver disease
  • smoking, and heavy alcohol intake
  • certain medicines, including some steroids and diuretics

What Usually Helps Bring It Down

Where a result comes back high, the first steps are almost always the ordinary ones, and they work:

  • replacing fried and packaged food with home-cooked meals, and refined grains with whole ones
  • adding soluble fibre — oats, beans, lentils, fruit and vegetables
  • moving for about 150 minutes a week, which can be a brisk half-hour walk five days a week
  • losing a modest amount of weight if you are carrying extra, since even 5–10% makes a measurable difference
  • stopping smoking, which improves several cardiac markers at once
  • bringing blood sugar under control if you have diabetes or prediabetes
  • taking cholesterol-lowering medication if prescribed, and taking it consistently

Apo B usually responds within eight to twelve weeks of a genuine change, which is why repeat testing is often scheduled around three months out.

How Is the Test Performed and Prepared For?

A small blood sample is taken from a vein in your arm using a fresh, sterile needle. It takes under a minute and feels like a brief pinch. You may be asked to press on the spot for a minute afterwards.

Fasting for 8 to 12 hours is preferred, which is why most people book a morning slot and skip breakfast. Plain water is fine and you should keep drinking it. Take your regular medicines as normal unless your doctor has specifically told you to hold them. If you are having this test alongside a lipid profile or a blood sugar test, the same fasting window covers all of them.

The sample can be collected at your home in every city Tapadia serves, or you can walk into any of our centres. Results are usually available the same day through the patient portal.

How Often Should It Be Repeated?

If your result is in range and you have no particular risk factors, once a year alongside your regular health check is usually enough. If the result was high, if you have diabetes or heart disease, or if you have just started or changed cholesterol medication, your doctor will typically want a repeat at around three months to see which way it is moving. Testing more often than that rarely adds anything, because the underlying biology takes weeks to shift.

What This Test Does Not Tell You

Apo B counts particles. It cannot show whether any of them have already lodged in an artery wall, how narrow a particular artery has become, or whether a blockage exists today. Those questions need imaging — a 2D Echo, a treadmill test or a CT angiogram, depending on what your doctor is looking for. Think of Apo B as a measure of pressure on the system over time rather than a snapshot of damage already done.

Frequently Asked Questions (FAQs)

  1. What is Apo B in plain terms? It is a protein carried by every cholesterol particle capable of blocking an artery. Because each particle carries exactly one, measuring Apo B counts them.
  2. How is this different from my usual cholesterol test? A cholesterol test measures how much cholesterol is being carried. Apo B counts how many particles are carrying it. When those two disagree, the particle count is the better guide to risk.
  3. Do I need to fast? Fasting 8 to 12 hours is preferred, particularly if you are also having a lipid profile. Water is fine, and you should take your usual medicines unless told otherwise.
  4. Can I have it collected at home? Yes. It needs only a blood sample, so home collection is available across all cities we serve.
  5. What does a high Apo B mean for me? It points to more artery-penetrating particles and therefore higher long-term cardiac risk. It is not a diagnosis — your doctor will read it with your other results before deciding anything.
  6. My LDL cholesterol is normal. Why has my doctor asked for this? Because the two can disagree. Normal LDL with a high particle count is common in people with diabetes, high triglycerides or extra weight, and Apo B is the test that reveals it.
  7. Will it show whether I already have a blockage? No. It measures risk, not damage. Detecting an existing blockage requires imaging such as a 2D Echo, treadmill test or CT angiogram.
  8. Can I lower it without medication? Often yes, at least partly. Diet, regular activity, weight loss and stopping smoking all reduce Apo B. Whether that is enough depends on your starting point and your doctor’s judgement.
  9. How soon after a change should I retest? Around three months. Apo B responds over weeks, not days, so testing sooner usually shows nothing meaningful.
  10. Should I test Apo A1 at the same time? It is worth discussing. Together they give the Apo B / Apo A1 ratio, which many doctors find more informative than either test on its own.

Who should take this test

  • Your LDL cholesterol looks normal but your doctor still suspects heart risk
  • A parent, brother or sister had a heart attack or stroke at a young age
  • You have diabetes, high blood pressure or carry extra weight around the middle
  • You smoke, or you have recently stopped
  • You have started cholesterol-lowering medication and want to see if it is working
  • You are having a yearly heart or full-body check
